Schedule: Variable shifts - 8-hour shifts (2nd shift)
Pay: $28 – $33 per hour

Job Summary
The Pharmacy Medication Reconciliation Technician is responsible for obtaining medication histories from patients and supporting medication reconciliation between home medications and inpatient orders. This role helps identify discrepancies, communicates issues to prescribers, and supports safe and accurate medication management throughout hospitalization and discharge.

Key Responsibilities
•             Obtain and document accurate patient medication histories
•             Assist physicians and pharmacy staff with medication reconciliation
•             Identify and communicate medication discrepancies during admission and discharge
•             Support high-risk patient medication reconciliation processes
•             Analyze and document medication reconciliation data for compliance and outcomes tracking
•             Assist pharmacy team under direction of senior or lead technicians
•             Perform additional pharmacy-related duties as assigned

Qualifications of the Pharmacy Technician:
•             Active California Pharmacy Technician License (Required)
•             Pharmacy Technician certification preferred
•             High School Diploma or GED preferred
•             Prior pharmacy or hospital experience preferred
•             Strong attention to detail and understanding of medication processes
•             Experience with Epic EMR preferred
•             Ability to work in a fast-paced inpatient pharmacy environment

Pay Details: $28.00 to $33.00 per hour
