👩‍⚕️ Your Impact as Patient Care Manager

In this role, you will:

✔ Lead & mentor clinicians to deliver exceptional home health care

✔ Oversee OASIS documentation accuracy & compliance

✔ Drive quality outcomes, audits, and performance improvement initiatives

✔ Ensure alignment with Medicare guidelines & regulatory standards

✔ Collaborate with physicians, patients & families on care plans

✔ Foster a culture of clinical excellence, accountability, and compassion

✔ Support staff growth through coaching, training, and development

✅ What You Bring

• Active RN License

• Home Health experience REQUIRED

• ✅ Strong OASIS knowledge/experience (highly valued)

• 2+ years of leadership or supervisory experience preferred

• Deep understanding of Medicare & compliance standards

• Passion for leading teams and improving patient outcomes

Pay Details: $10,000.00 to $11,000.00 per year
