Licensed Practical Nurse (LPN/LVN) – Home Health

Location: Grants Pass, OR
Sign-On Bonus: $10,000
Schedule: Full-time, Monday–Friday
Position Type: Field-based

We’re seeking an experienced LPN/LVN to provide compassionate, one-on-one nursing care to patients in their homes. This is an excellent opportunity for a nurse who values independence, meaningful patient relationships, and a consistent weekday schedule.

Responsibilities

• Provide skilled nursing visits according to physician-prescribed plans of care

• Monitor patients’ conditions and communicate changes to the clinical team

• Educate patients and caregivers regarding medications, treatments, and care needs

• Maintain accurate and complete clinical documentation

• Collaborate with physicians, nurses, therapists, and other care team members

• Travel throughout the assigned territory to provide in-home patient care

Qualifications

• Graduate of an accredited practical or vocational nursing program

• Active Oregon LPN or LVN license

• At least one year of clinical nursing experience

• At least one year of home health experience

• Current CPR certification

• Valid driver’s license and ability to travel throughout the coverage area

• Strong communication, clinical judgment, and documentation skills

Pay Details: $32.00 to $40.00 per hour
