Position: Home Health Physical Therapist (PT)

Location: Elizabethtown, PA

Coverage Area: Dauphin County, PA

Schedule: Monday–Friday | 8:00 AM – 5:00 PM

Employment Type: Direct Hire

Salary: $108,000 - $132,000 annually

🌟 What You'll Do

As a Home Health Physical Therapist, you'll make a meaningful difference every day by helping patients safely recover in the comfort of their own homes.

Responsibilities include:

✔ Perform comprehensive physical therapy evaluations

✔ Develop individualized, evidence-based treatment plans

✔ Deliver one-on-one therapeutic interventions to improve strength, balance, mobility, and functional independence

✔ Educate patients and caregivers on home exercise programs and injury prevention

✔ Monitor patient progress and modify treatment plans as needed

✔ Collaborate with physicians, nurses, and interdisciplinary care teams to achieve exceptional patient outcomes

✔ Maintain accurate clinical documentation and ensure regulatory compliance

 

✅ Qualifications

• Current Pennsylvania Physical Therapist license

• Master's Degree in Physical Therapy from a CAPTE-accredited program

• Valid CPR Certification

• Valid driver's license with the ability to travel throughout the assigned coverage area

• Home Health or Community-Based Physical Therapy experience preferred

• Strong clinical assessment, communication, and patient education skills

Weekend Position Details

📍 Elizabethtown, PA

💰 Salary: $98,000 - $120,000 annually

📅 Flexible Schedule:

• Friday–Sunday OR

• Saturday–Monday

Minimum of 9 months of Physical Therapy experience required.

Pay Details: $98,000.00 to $132,000.00 per year
