Exciting LMSW opportunity working with seniors in the Omaha area! Come join a growing innovative team that is looking to create a great experience for seniors. Great culture with strong core values, competitive salary ($60-$80k) and great benefits. Must be willing to travel around the territory.

Some of the Duties/Responsibilities:

1.      Develop relationships and collaborate with primary care tea to conduct social services assessments to determine the appropriate needs for each member.

2.      Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers.

3.      Navigate managed care plans for community services and programs.

Some of the Required Skills/Abilities:

1.      Experience working with patients in need of social services.

2.      Experience with assessments and planning for social services.

3.      Working knowledge of different diagnosis of common health conditions.

Education and Experience:

1.       Fully licensed LMSW in NE.

2.       Master's degree in Social Work.

3.       1+ years' experience in clinical social work

4.   A problem-solving orientation and a flexible and positive attitude

5.    Mission driven and motivated to join an organization that will transform the way we deliver accessible client excellent care to seniors.

6.       Proficient PC skills.

7.     Fluency in Spanish or other language spoken by people in the communities we service.

Pay Details: $60,000.00 to $80,000.00 per year
